Do You Qualify For Implant Treatment?
If you are concerned about travelling to Belize for your implant treatment, only to find out that you do not qualify, we have created these four steps for you to follow to enable us to evaluate your case . This way you can schedule your visit to Belize with complete peace of mind.
To enable us to determine whether you qualify for dental implant treatment in Belize, we need information from you. Please follow the steps below on how to obtain what we need to evaluate your case and how to submit the information to us:
Your personal information is safe with us and will not be shared with third parties other than the Implantologist and clinic staff.

Contact Us. That way we have your name and contact details for future communication.
In the message section, please give us a short summary of what you wish to achieve with the implant treatment.
2.
Have your CBCT and intraoral scans taken by a clinic at home.
When you visit the clinic, please provide them with this document that you have to print before the appointment. This document will give them clear instructions on what we need and how they should send it to us.

Please complete the Medical and Dental History form and email it to us. This form is of great importance, so please provide all the information accurately.
The form is a fillable form that you can download and fill out on any electronic device you have. Please ensure that your form and the information on the form are visible and accurate. Also, note that the form must be signed (an electronic signature is acceptable). If you don’t have a PDF reader you can click one of the links below to access one on your computer or look for the Adobe Acrobat Reader or Foxit PDF Editor in the App Store/Play Store on your phone.

Once we have received the needed information and reviewed your case, we will send you an email to schedule a Zoom call to discuss the treatment option/s with you.
Treatment Approved?
You are now ready to schedule the surgery and to make travel arrangements for your visit.
